Vendors shall maintain, and keep at the family day home, written or electronic
information for each enrolled child, which shall include: Proof of the child's identity, Name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.

Child #3's record did not contain proof of identity.
Child #2 and Child #3's records did not contain the name, address or phone number for at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: Vendor will request required information for parent today at pick up.
