Inspector Notes

An in-person annual subsidy inspection was initiated by licensing staff and concluded on March 19, 2024, between the hours of 12:30pm to 2:00pm. There were four children in care and no children living in the home under the age of 12 present with the vendor. The inspector reviewed four children?s records and four adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.

Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the Violation Notice issued to the vendor.

Based on record review, documentation review, and interview, the vendor did not ensure that the applicant, household member, and any caregiver involved in the day to day operations of the family day home obtain repeat criminal history records check results every five years.
Evidence:
1. Caregiver #1's records did not have documentation of a repeat criminal history records check results. Documentation of a criminal history record check results was dated 09/26/2018.
2. Household Member #1's record did not contain documentation of a repeat criminal history records check results. Documentation of the criminal history record check results was dated 09/26/2018.
3. Household Member #2's record did not contain documentation of a repeat criminal history records check results. Documentation of the criminal history record check was dated 09/26/2018.
4. Caregiver #1 stated that paperwork for the criminal background check was submitted to the proper agency on 08/25/2023 to obtain appointments for the criminal history check. The appointments for the criminal background checks were not received.
Plan of Correction: Although I am very disappointed in the end result, I feel as I was in compliance based on dated documentation. I first submitted my information in August 2023 and then again in September 2023. Upon receiving notification from a different agency, I then followed up again in reference to my fingerprints and background. I feel this is not my personal issue and I am deeply dissatisfied with the determination. However, Household Member #1 completed their fingerprint today 3/19/2024. Caregiver #1 and Household Member #2 will complete theirs tomorrow 3/20/2024. Our appointments are scheduled beginning at 1:00 pm. I now understand that I have to continue to press the issue and follow up in more depth to ensure all paperwork is received and documented in a timely fashion. I have never been in violation of anything ever in my life especially pertaining to my childcare and I can assure you this will never happen again.
