The family day home is required to have household members undergo a background check every five years.

The record for household member #1 did not have a updated central registry. The central registry finding was eight months overdue.
Plan of Correction: The provider will request the Central Registry finding and forward them to the inspector once the results are received.

include the department's health and safety update course.
