Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 06/08/2022. At the time of inspectors' arrival there were 2 school aged children in care with 2 staff members. Children were observed interacting with staff and participating in free play. A sample of 2 children's records and 2 staff records were reviewed. There were no injury reports or medications for review. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review it was determined the center did not ensure that each child had a physical examination by or under the direction of a physician within the required timeframe.
Evidence:
1. The record for child #1 did not contain documentation of a physical exam.
2. Staff confirmed that this documentation was not available during this inspection.
Plan of Correction: Staff will re-request documentation of a physical.
