Inspector Notes
A non-mandated monitoring inspection was conducted on October 16, 2023, from approximately 10:15 AM to 11:08 PM. The inspector reviewed compliance in the areas of administration, physical plant, programs, special care and emergencies, and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations
7Evidence:
During the October 16, 2023 inspection, the following was documented:
1. At approximately 10:15 a.m., two licensing agents approached the entrance to the center and rang the doorbell. Staff #1 answered the door and denied the licensing agents the opportunity to enter the center. Staff #1 reported that she was busy with online training and it was not a good day. The director/owner came to the door and told the licensing agents that they could not come in the center. While one of the licensing agents was on the phone with local law enforcement, the director/owner told the other licensing agent that she would allow five minutes inside, but she was not having it if 30 minutes was needed. The director/owner allowed the licensing agents to enter the center at approximately 10:25 a.m.
2. While in the preschool, a licensing agent was obtaining information from center staff. The director/owner directed the licensing agent to direct questions to her.
3. After entering the infant room, the licensing agents were asked how long they would be in the room with their shoes on. The licensing agents stepped outside of the room to remove their shoes. When the licensing agents started to return to the infant room, the director/owner shut the door to the infant room and directed them to the office to view infant schedules.
4. While walking to the office, the director/owner shut the door to the kitchen. The licensing agents requested access to the kitchen. The director/owner denied access to both licensing agents. Instead, the director/owner entered the kitchen, shut the door and opened the bi-fold blinds for the serving window. The director/owner confirmed that children bring meals from home. The director/owner showed one lunchbox with a child?s initials when asked if the food was labeled. The director/owner reported that the food inside of the lunchbox was dated, but refused to show the inside of the lunchbox and immediately closed the bi-fold blinds to the serving window.
5. A full monitoring inspection was not conducted and concluded at approximately 11:08 a.m.
Evidence:
In the infant room, one crib mattress had exposed foam along one side within reach of children in care. Two additional cribs had plastic wrap around the mattresses within reach of children in care presenting a choking hazard.
Evidence:
The four cribs in the infant room were not labeled in a way to identify which child used the crib. At the time of inspection, there were two children in care in the infant room.
Evidence:
Staff #3 was observed changing a diaper. After the diaper change, staff #3 did not wash the hands of the child. Staff #3 sat down in a chair and played with the infant. Staff #3 did not wash her hands with soap and running water.
Evidence:
1. After completing a diaper change, staff #3 finished diapering a child and sat down in a chair with the child. The staff member did not clean the diapering surface with soap and at least room temperature water or sanitize after changing the child?s diaper.
2. A bottle that the child was drinking was lying on the diaper mat while the child was being changed.
Evidence:
Multiple lunch boxes were observed in the kitchen serving window. The director/owner stated children bring food from home. The inspector asked if they were labeled with the name and date. The director/owner pointed out one lunch box with the initials of the owner. When asked if the lunch was dated, staff #2 responded that the date was inside the lunchbox. The inspector asked to see where it was dated and the director/owner refused.

Evidence:
One prepared bottle was observed sitting on the changing table in the infant room. The bottle was not labeled with a name or date. Staff #3 stated that the child was the only one on a bottle, so she knew who it belonged to.
