Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on May 8, 2025. The on-site inspection began at 10:00 am and ended at 12:25 pm. The inspector reviewed compliance in the areas listed above. There were 14 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on May 19, 2025. A POC submitted after this date will not appear on the public website.

6The center did not request a search of the child abuse and neglect registry or the out of state sex offender search from one state for Staff #2, who has been employed for more than two months and was working with children at the time of the inspection.
There was no physical examination in the record of child #1, who had been enrolled for two months child #2 who had been enrolled for 4 months.
Staff #2 is currently working and has a TB screening that was dated 2 years prior to their first day of employment.
Three infant bottles with one with water and two with formula were not labeled with the child?s name or date. Two infant bottles with formula were labeled with a name but was not dated.
The record for Child # 2, #3, and #4, enrolled for over two years, was missing an annual update signed by the parents documenting that the information in the child?s file is up to date.
