The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
1. The carpet in the Three?s and Four?s classroom was ripped posing a tripping hazard.
2. In the two?s classroom, there was a child size chair torn exposing the inside cushion materials.

Plan of Correction: The center has repaired the carpet seams to prevent a tripping hazard. The center has removed the chair with the torn cushion and replaced it with a new chair.
