Inspector Notes

An unannounced Renewal Inspection was conducted on September 5, 2023. Upon arrival at 11:30 AM, there were a total of 6 children under the care of 2 staff present. The Licensing Inspector reviewed 3 children?s records and 3 household/staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Licensee failed to keep a written record of children in attendance each day.

A written record of attendance was unavailable.

Based on record review, the Licensee failed to ensure each child's record contains required information.

EVIDENCE:
Licensing Inspector reviewed 3 children records. Record for Child #2 and #3 did not include the first date of attendance.
