Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The center must request a search of the central registry prior to the employee's first day of employment. The record for Staff #4 , who has been employed for a week, did not contain documentation that a central registry search had been requested before hire.

Within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff. The record provided for Staff #3, who was employed for six months, did not include documentation of completion of the Virginia Preservice training.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition. There was peeling paper and exposed drywall in three locations in the Pre-K 1 classroom. There was a baseboard peeling off the wall in the Prek 4B classroom.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. There were at total of 12 electrical outlets that did not have protective covers in multiple areas including the hallway, bathroom and classrooms.

Hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. There was a container of bleach water spray in an unlocked cabinet in the Prek 3A classroom.

The vendor shall ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment. There were three Little Tykes slides on the playground that did not have sand or other cushioning materials under the equipment. Children were observed playing on the slides.

The vendor shall maintain a record of the dates of the practice drills for one year. For vendors offering multiple shifts, the simulated drills shall be divided evenly among the various shifts. There were no records of emergency evacuations, shelter-in-place and lockdown drills for the 2023-2024 school year.
