Inspector Notes

An unannounced focused monitoring inspection was conducted today with the provider from 2:05 to 2:45 pm. Four children were present (4 months, 10 months, 2 years and 3 years of age) for a total of 12 points. The children were sleeping during the majority of the inspection.
Four children's files were reviewed. There were no medications on site. The areas of non compliance are outlined on the violation notice.

Standard 8VAC20-800-100-A
Based on a review of children's files and an interview with the provider, it was determined that child A ( enrolled on 3/6/23) did not have a physical on file within 30 days of enrollment.
Plan of Correction: I will get the physical from the parent.
Standard 8VAC20-800-520-C
Based on observation, it was determined that child B ( 10 months of age) was sleeping in a crib where two mattresses was stacked on top of each other which resulted in the second mattress failing to fit snugley next to the crib so that more than two fingers could be inserted between the mattess and the crib.
Plan of Correction: The second mattress was removed and will be returned to the parent.
Standard 8VAC20-800-60-B
Based on a review of 4 children's files and an interview with the provider, it was determined that two children (A &B) did not have their insurance policy number listed. This is requirement at the time of enrollment. Child A and B enrolled on 3/6/23.
Plan of Correction: I will get the insurance policy numbers for the files.
Standard 8VAC20-800-90-A
Based on a review of four children's files and an interview with the provider, it was determined that two children ( Child A and B) did not have immunization records on file, which is required before a child may attend the family day home.
Plan of Correction: I will get the immunization records from the parent.
