Inspector Notes

An unannounced renewal inspection was conducted today with the provider from 12: 35 pm to 1:15 pm. The provider was present with a two year old child who napped during the inspection.
The provider does not transport, there have been no injuries/accidents and there are currently no medication on site for any of the children. Three household member's files and three children's files were reviewed.
Please notify your Licensing Inspector upon submission for the fingerprint background check for Person #1. You must submit for this background check within 10 days. Once the background check is completed, please forward a copy to your Licensing Inspector.

Based on interview and a review of the application, it was determined that a household member had not obtained a fingerprint criminal background check within 30 days of residency.
Evidence:
Person #1 came to reside in the home in July 2023. There was no fingerprint background check on file.
Plan of Correction: We will get the background check.

Based on interview and a review of the application, it was determined that a household member had not obtained a tuberculosis test/screening at the time of residency and prior to coming in contact with children.
Evidence:
Person A does not have a tuberculosis test/screening on file.
Plan of Correction: We will get the tuberculosis test/screening.

Based on interview, it was determined that an assistant did not have 16 hours of annual training.
Evidence:
Person #2 has been an assistant since March 2022 and has not obtained any of the 16 training hours from March 2022 to March 2023.
Plan of Correction: We will get the 16hours of annual training.
