Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 5/20/24. The on-site inspection was conducted from 12:45 pm to 1:50 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present with one caregiver. The inspector reviewed 5 children?s records, 1 caregiver record and 2 household member's records. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A physical is required for each child within 30 days after the first day of attendance.
There was no physical on file for Child A, 4 months after the child enrolled.

Current certification in first aid and CPR are required.
Caregiver A's First Aid and CPR certification expired over 4 months ago.
Plan of Correction: I will obtain up to date First Aid and CPR.

Each caregiver and adult household member shall have a current Tuberculosis test/screening every two years from the date of the first Tuberculosis test/screening.
