Inspector Notes

An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted on 8/28/2024 in conjunction with a renewal inspection (see that inspection report for details). The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. 1 staff record was reviewed for documentation of the mandatory Virginia Preservice, Health & Safety Update training, and current certification in First Aid and CPR. The violation is outlined in the notice.

Evidence: The documentation that was observed had no documented lockdown procedures practiced and provider stated that there were no lockdown procedures practiced.
