Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/24/2025. The on-site inspection began at 12:25pm and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 16. The inspector reviewed 4 children?s records and 3 caregiver/household member(s) records onsite.

This inspection included document, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The provider shall maintain an up-to-date record at the family day home for each enrolled child with the required information.

Child A?s record was missing name, office address and telephone number of the child's physician; name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; name and policy number of the child's medical insurance, if applicable; Written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
