Inspector Notes

An unannounced monitoring inspection was conducted on August 30, 2023 from 10:15 am-12:10pm . At the time of entrance, 52 children were in care with 11 staff members present. Additional staff was present to assist. The sample size consisted of 5 children?s records and 5 staff records. Children were observed playing with manipulative, puzzles, tummy time and outdoor play time. Three medication documentation were reviewed.

Based on record review and staff interview, the licensee did not ensure each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.

Evidence:
Staff #4 (employment date 03/13/2023) there was no documentation of TB result able to be provided during the inspection.
Staff #5 (employment date 08/21/2023) there was no documentation of TB results able to be provide during inspection .
