Inspector Notes

A unannounced mandates monitoring inspection was initiated on April 24, 2024 from 9:25am - 11:50 am. The inspection concluded on April 24, 2024. There were 46 children with 11 staff present and additional staff was present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care
and emergencies and nutrition. A total of six children records and six staff records were reviewed. The children were observed participating in art activiites, story time, freely playing in station activities and diapering. The outside area was not inspected.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations

Based on record review and staff interview, the licensee did not ensure the authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Evidence:
1- Child #1's medication authorization form signed by parent was not renewed every 10 working days.

2- Child #1 medicaiton authorization form for long term was not signed by child's physician.

3- The authorization was good for 10 working days. The medication authorization form for child #1 started February 23, 2024 . Ten working days was March 8, 2024 .

3- The center did not return the medication with in 14 days or have parent fillout another medication authorization for 10 days or get a long term med
ication form signed by parent and physician.

4- Fourteen days was March 28, 2024 and medication was still at the center on April 24, 2024. .
Plan of Correction: The director will send the parent a new form home with parent to be signed by the doctor.
