Inspector Notes

An unannounced monitoring inspection was initiated on July 24, 2024 and completed on July 24, 2024 in response to a self-report (IROMT)received by the licensing office on July 12, 2024 relating to outside medical treatment. The inspector reviewed two children?s records and one staff record on-site. This inspection included document review, tour of the facility, interviews and observations.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
