Inspector Notes
An unannounced inspection was completed on 06/11/2024 in response to a complaint received by the licensing office on 06/10/2024 relating to allegations of programs and daily activities. This inspection included document review, observations at a local park during a field trip, vehicle child restraint inspection, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to special care and provisions and emergencies and special services and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/04/2024. A POC submitted after this date will not appear on the public website.
Violations
3Evidence:
On 06/11/2024 during the inspection of the passenger van used during a field trip, licensing inspector requested to review the document prepared by the center containing local emergency contact information, potential shelters, hospitals, evacuation routes that pertain to each site frequently visited or routes frequently driven by center staff for center business such as field trips. This documentation for the field trip to the library and playground on 06/11/2024 was not available for review.
Evidence:
1. During an inspection on 06/11/2024, the Licensing Inspector observed staff transporting children in a 15-passenger van. The Licensing Inspector stopped the van as it was exiting the park enroute back to the center. The inspector observed ten children and two staff in the van. Out of the ten children (ages 21 months-5 years), only two of the children were restrained in either a child restraint device or a lap seatbelt. The provider failed to ensure that during transportation of children that the Virginia state statutes about safety belts and child restraints are followed as required by the Code of Virginia.
2. The van used for the field trip to the library and playground on 06/11/2024 did not have emergency numbers, the center?s name/address/phone number nor a list of the names of the children being transported in the vehicle.
Evidence:
On 06/11/2024 S1stated that the children were on a field trip to the local library and when the licensing inspector was in route to the library the S1 advised that the children were at the local school playground. The center did not have a schedule of the trip?s events and location posted and visible at the center site.
