Repeat Violation
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
C3, under the age of 2, and enrolled for over 19 months did not contain additional immunizations due within six months of enrollment.

Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for child, C4 was not signed by a physician.

The center shall ensure that program leaders meet all the required qualifications.
The records for staff S2 and S did not have documentation to demonstrate how the staff meets program leader qualifications.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of the children in care. S6 obtained 12 hours and S7 obtained 11 hours.

The center is required to provide training on the center?s emergency preparedness plan annually and each time the plan is updated.
S7 reported the annual review of the emergency preparedness plan has not been completed.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
The diaper changing pad was torn in the infant room.
Multiple electrical outlet and power strip in the multipurpose room were missing outlet covers.

Hazardous substances must be in a locked place that prevents access by children. In the JWW classroom and Multipurpose room, hand sanitizer was accessible to children with the warning label ?Keep out of reach of children?.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

Licensing inspector observed an infant asleep in a crib in the infant room and no staff were observed in the building or infant room. Interviews were conducted and staff reported they were outside the building doing a lesson on fall leaves with the children for approximately 10-15 minutes and that they were periodically looking in the classroom where the 3-month-old child was sleeping.

The center is required to update the injury prevention procedures at least annually based on documentation of injuries and a review of the activities and services.
S7 reported the review of the injury prevention procedures has not been completed.

Cots used by children shall have linens consisting of a top cover and a bottom cover.
A child was observed on a cot in The Christian room that did not have a bottom cover.

Each center shall maintain emergency evacuation drill records that contain all the required information. The drill records dated 02/26/2024, 03/27/2024, and 04/26/2024 did not document the weather conditions during the drills.

Injury reports for children should be either signed by staff and a parent or by two staff. The injury report for C4 dated 08/19/2024 was neither signed by staff and a parent or by two staff.

When food is brought from home the container must be clearly labeled with the child?s name.
Multiple thermos bottles in the Raider classroom were not labeled with the children?s names.

Repeat Violation
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all the required information.
The record for child, C3, enrolled for over 19 months was missing the annual confirmation that required information in the child?s record is up to date.
The record for child, C4, enrolled for 2 months/25 days was missing information on allergies, health problems, previous child day care attended by child, and the address of a second emergency contact if a parent cannot be reached.
