Inspector Notes

An unannounced, on-site monitoring inspection was completed on 12/18/2024, as a part of provisional licensure period. The on-site inspection began at 10:35 a.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 22 children present and 4 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 01/20/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
Feces was observed on the diaper changing pad in the infant room.
Electrical outlets in the infant room were missing outlet covers.

The required ratio for children ages 2 years old is 1 staff to 8 children. Licensing inspector observed 1 staff supervising 8 children in the JWW Room during lunch; however, the youngest child in the room due to classes being combined was not yet 2 years old causing the class to be out of the required 1-5 ratio.

The required ratio for children ages 3 years old up to school age eligible is 1 staff per 10 children. Licensing inspector observed 1 staff supervising 12 children in the Raider?s room. The room came back into ratio when a staff member returned to the room from a restroom break.

The center shall maintain and keep at the center a separate record for each child enrolled that contains all the required information.
The records for children, C1, C2, C3, C4, and C5 enrolled for over 12 months was missing the annual parental confirmation that required information in the child?s record is up to date.
The record for child, C2, was missing information on allergies and health problems.
