The center is required to obtain a completed sworn statement prior to the employee's first day of employment. S2 has been working for 9 months did not have a completed sworn statement.

Repeat Violation
The center is required to obtain background checks from any state in which the individual
has resided in the preceding five years. S2, employed for more than 9 months, did not
have documentation of requesting a central registry check or sexual offender check
from one state.

Repeat Violation
The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. S2 is currently working and has a TB screening that was dated 10 days after their first day of employment.

Repeat Violation
A tuberculosis (TB) screening is required every two years from the last screening. S1 updated screening was due 39 days ago. The updated screening has not been completed.

Prior to working alone with children and within seven days of the first day of employment, staff shall complete orientation training required by 8VAC20-780-240-C. The employment file for S2 employed for more than 9 months did not contain an orientation form.

The center shall ensure that areas and equipment of the center, inside and outside,
are maintained in a clean, safe and operable condition.
1. The refrigerator in the infant room had a spilled white substance in the compartment holding children?s food.
2. The safety straps on several infant feeding chairs were caked with dried and sticky food debris.

Children under 10 years of age shall always be within actual sight and sound supervision of
staff. The inspector arrived at the center at 12:15 p.m. and observed 3 staff members congregating in the parking lot. Upon entering the building, the licensing inspector inspected the Sigma classroom and observed no staff in the room with 6 children, aged 3- years-old, and napping.

Staff shall wash their hands with soap and running water after coming into contact with body fluids. A staff member in the JWW room was observed wiping multiple children?s hands and faces after snack time without washing her hands between assisting the children and then serving seconds of snacks.

Documentation of at least 2 references to character, reputation, and competency are to
be checked prior to employment. S2, who has been actively working at the center for more
than 9 months, did not have any completed reference verifications on file.
