An unannounced, on-site monitoring inspection was initiated on 8/6/2024 and completed on the same day , as a part of the regular licensing period. The on-site inspection began at 12:10 and ended at 1:15. The inspector reviewed compliance in the areas listed above.
There were 4 children present and 2 caregivers; the point total was 9. The inspector reviewed 3 children?s records and 3 household member records during the inspection.

Evidence; The attendance record was left blank for 8/5/24.
Plan of Correction: Fixed during the inspection. I will make sure I do attendance every day.

Based on the review of three children's records, the provider failed to get annual updates from parents.
Evidence: reviewed 3 children's records, child 2 and 3 need annual updates in the files.
