During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 1, 2025 and completed on April 1, 2025. The on-site inspection began at 1:45 PM and ended at 3:05 PM. The inspector reviewed compliance in the areas listed above. There were 15 children present and four staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 11. 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-440-A
Cribs, cots, rest mats or beds shall be provided for children during the designated rest period

On 4/01/24, at 1:55 PM, four preschool children were resting directly on the carpet during the designated rest period. The center?s daily schedule documents that nap time is from 12:30 PM to 2:30 PM. Staff #4 confirmed that it was the designated rest period and that the preschool age children had just finished watching a movie.
Plan of Correction: Moving forward, we will ensure that all preschool-aged children are provided their designated cots and are on them during the full rest period, regardless of any changes to the before and after care schedule, including during school breaks. Staff will be reminded of rest time protocols, and we will adjust scheduling and space usage as needed to maintain a calm and uninterrupted rest period for preschoolers.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

At approximately 1:45 PM on 4/01/25, there were 15 children present in the center, but only 13 were marked present on the attendance record. The administrator confirmed that two of the 15 children were not signed in on the correct date.
Plan of Correction: To ensure compliance moving forward, staff will actively monitor the sign in and sign out process each day. We will kindly and clearly direct parent to the correct location on the attendance sheet and physically point to the appropriate date and time section to ensure accuracy. Additionally, reminders will be posted near the sign in area to reinforce this process. We are committed to maintaining accurate attendance records in accordance with licensing requirements.
