Violations

2
Standard 8VAC20-800-240-A
Based on observation, the provider did not ensure that the home is maintained in a safe condition.

Evidence: While walking through the home, one of the stairs had a piece move away from the stair. The provider proceeded to place it back, but it was easily moved again.
Plan of Correction: We will fix the stairs
Standard 8VAC20-800-800-B
Based on record review, the provider did not ensure that the emergency plan is updated as needed.

Evidence: The emergency preparedness plan has not been updated for the new home. Items in the plan still reference the previous address and evacuation locations for that space. New information is needed for the new home.
Plan of Correction: We will create a new emergency plan.
