An unannounced, on-site initial inspection was conducted on 11/7/2024. The on-site inspection began at 9:50am and ended at 10:55am. The inspector reviewed compliance in the areas listed above. There were no children present. The inspector reviewed 4 caregiver/household member(s) records. This inspection included document review, interviews, observations, and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
