An unannounced annual inspection was conducted on 12/13/2023 from 10:18 am to 11:15 am. There was 1 child present and 1 direct caregiver.
The home, 1 caregiver record, 1 child record, 2 household members records, evacuation drills, emergency supplies, and policies were inspected. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Based on interview and record review, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

3. Child #1 is missing the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.
4. Child #1 is missing the name, address, and telephone number for each custodial parent's place of employment or school attendance
5. Child #1 is missing the child's full name, nickname (if any), sex, address, and birthdate.
6. Child #1 is missing the name, home address, and telephone number for each parent who has custody.
7. Child #1 is missing written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection for the provision of medical treatment on religious or other grounds

Based on observation, an infant, toddler, or preschool child who falls asleep in a play space other than his own crib, cot, mat, or bed was not moved promptly to their designated sleeping space if the safety or comfort of the infant, toddler, or preschool child is in question.

1. Child #1 (birthdate unknown- 4-month-old infant) was sleeping in a car seat when the inspector arrived at the home approximately at 10:18 am.
