Inspector Notes

An unannounced two month inspection was conducted today. A total of 20 children with 10 staff were in classrooms today. A total of 4 classrooms were occupied. Observation of all 4 classrooms was made. The children were observed participating in a gross motor activity in the hallway due to the weather. The 3 year old class was engaging in an eye spy activity and transitioned to a circle book reading activity. Diapering and handwashing was observed in the infant room and morning snack was observed in the Red Panda Room All medication onsite was reviewed. Staff were observed sitting with children while they ate and positive interaction was observed in all four classes. A total of 4 children's records and 4 staff records were reviewed. Based on staff record review, one background check was not obtained prior to hire.

Evidence: Staff #1 has resided outside the state of Virginia in the last five years. An out of state criminal history search was not obtained prior to hire.
Plan of Correction: Complete criminal history search and forward results to inspector within 10 working days.
Standard 8VAC20-780-130-A
Based on child record review, one child did not have documentation of immunizations before attending the center.

Evidence: Child #1 (start date: 04/11/2022) did not have record of immunizations.
Plan of Correction: I will ask mom to submit the immunization record.
Standard 8VAC20-780-160-A-2
Based on staff record review, one employee did not have a Tuberculosis (TB) screening completed within 30 calendar days of the date of employment.
Evidence: Staff #2 (start date: 4/4/2022) had a TB screening dated 01/14/2022.
Plan of Correction: All new hires will have a TB screening prior to hire and within 30 days.

Based on review of medication, one medication was not in the original pharmacy container with a pharmacy label.

Evidence: Medication for child #2 was not in its' original pharmacy container with a pharmacy label.
Plan of Correction: The parent will provide a pharmacy label.
Standard 8VAC20-780-570-B
Based on observation, one infant who cannot hold their own bottle was not held while being bottle fed.

Evidence: One staff was observed bottle feeding an infant while the child was laying down on a pillow.
Plan of Correction: Staff will be reminded to hold infants during bottle feeding for those that cannot hold their own bottle.

Based on observation, prepared infant formula was not dated.

Evidence: Child #3's bottles were not dated.
Plan of Correction: Staff and parents will be reminded that prepared bottles need to be labeled with the date.
