Inspector Notes
A Renewal inspection was conducted onsite today, A total of 22 children with 10 staff were present in the classrooms. Observation of all 4 occupied classrooms was conducted. Infants were observed either being held by staff or crawling and playing in the program space, one infant was observed sleeping. One class was outside at the nearby park. The other two classrooms were either eating lunch or preparing to eat lunch. Handwashing procedures were observed as the children transitioned to the tables to eat. Staff in the Saola Class were observed sitting with the children and one staff observed feeding apple sauce to one child. A total of 5 children's records, 6 staff records and all medications and topical ointments and authorizations for each were reviewed. One repeat violation from the May inspection. Staff #2 was hired on 6/27/22, Staff #3 was hired on 4/18/22 and Staff #4 was hired on 5/9/22. The orientation training on file did not include training in First Aid and CPR.
Evidence:
The medication authorization for Child #6 did not include the duration or frequency for the medication.
Evidence:
Saline drops and 1 Epi Pen in the Red Panda Classroom was not labeled with the child's name, the dosage amount and the times to be administered.

An EpiPen in the Red Panda Class was not maintained in the original, pharmacy labeled container.
Evidence:
Staff #7 stated the Child #7 has received medication twice daily since April 2022. There was no record of medication administration to Child #7 since 8/3/2022.
Evidence:
Two bottles for 2 infants did not include the date on the label.
Evidence:
1. Staff #6 is a lead teacher. Staff #6's record did not contain documentation of lead qualifications.
2. Staff #4's record did not contain a second reference.
