Inspector Notes
An unannounced Monitoring Inspection was conducted today. A total of 72 children with 21 staff providing supervision was onsite. Observation of all 8 occupied classrooms was made. Staff interviews were conducted and all medication was reviewed. In the infant room, staff were observed diapering, actively engaged with infants on the floor and documenting daily progress. The cribs onsite were clean and adequately spaced apart. Proper cleaning and sanitizing of the diaper surface and hand washing procedures were observed. Artwork and projects were on display on all classrooms and directly correlated with the planned lessons. In the Red Panda Classroom paper plate suns and magic hands that were made from dried leaves were hung on the walls along with painted bumble bees and a board of feelings. Other items on display was a "My name is" and a lifesize "My body" drawing in which the children had colored today. Plans to glue dried pasta onto the drawing to create a skeleton will complete the project. A total of 7 children's records and 6 staff records were reviewed today. A new Director of record onsite since April and qualifications were previously reviewed. This inspection occurred between 11:20am and 2:15PM. Some violations were found and corrective actions were discussed. Thank you for your cooperation today.

3Evidence:
1. Staff #6 was hired in June 2023.
2. Staff #6 currently lives out of the state of Virginia and has lived in another state within the past five year.
3. Out of state criminal record search results were not obtained prior to hire.
Once the result is received, it will be forwarded to the Licensing Inspector within 10 working days for review.
Evidence:
Injury reports dated March, May and August 2023 did not include when and how Child #6's parent was notified.
