Inspector Notes

An unannounced monitoring inspection was initiated and completed on 06/10/2025 in response to a self-report received by the licensing office on 06/04/2025 relating to administration, staff supervision and special services. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/18/2025. A POC submitted after this date will not appear on the public website.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.
1. On 06/04/2025, Child #1 (age 4 years) was not within sight and sound supervision for an undetermined length of time.
2. One grouping of children went to the fifth floor for an activity. Child #1 was left behind on the fifth floor of the building while Staff #1 and #2 returned the grouping of children to their classroom located on the first floor where the child day center is located.
Plan of Correction: Faculty were re-trained on name-to-face and attendance procedures.
All classrooms procedures for transitioning students were audited.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures.

1. The center's own policies for identifying where attending children are at all times, including procedures to ensure that all children are accounted for before leaving a field trip site and upon return to the center was not followed on 06/04/2025.
2. The center's policy requires that staff complete a "face to name" identification of each child at every threshold. Staff #1 and #2 did not complete a "face to name" at six thresholds on 06/04/2025.
3. Child #1 was left on the fifth floor of the building during the time Staff #1 and Staff #2 was returning with a grouping of children to the first floor of the building.
Plan of Correction: Faculty were re-trained on name-to-face and attendance procedures.

Parental permission for field trips shall be secured before the scheduled activity.

Parent permission was not secured for 12 children on 06/04/2025 for a scheduled activity.
