Inspector Notes

An unannounced monitoring inspection was initiated and completed on 03/09/2026 in response to a self-report received by the licensing office on 03/03/2026 relating to staffing and supervision, program and administration. The inspector reviewed 1 child record and 4 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/18/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

Staff # 2, 3 and 4 had witnessed Staff #1's inappropriate interaction with Child #1 over approximately 3 months and failed to report the inappropriate interaction to management.
Plan of Correction: Entire staff will review Mandatory Reporting training at regular occuring staff meetings.
Standard 8VAC20-780-360-A
The variety of daily activities for all age groups shall be age and stage appropriate.

An activity provided by Staff #1 in one classroom was not age and stage appropriate for children.
1. Staff #1 instructed Child #1 to repeat inappropriate language in front of other children and staff on more than one occasion.
Plan of Correction: Staff #1 was terminated.
Entire staff completed Mandatory Reporting Training and attended a staff development meeting where the importance of ?If you see something say something? was reaffirmed. A link was provided to staff where they can report concerns anonymously to admin at anytime.
