Inspector Notes

An initial inspection was initiated on April 12, 2022 and concluded on April 12, 2022. The Administrator was contacted to conduct an on-site inspection to obtain measurements. The Inspector reviewed seven board members and program director qualifications.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation and interview, the facility did not obtain one working, battery operated radio in each building used by children.

Evidence:
The facility had a battery operated radio however staff were unable to get the radio to work.
