Inspector Notes
A renewal inspection was completed on December 22, 2022, at approximately 3:25 PM to 4:40 PM. There were eight children present, ranging in age from five years to 12 years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies and nutrition. A member of management stated there were no medications on site and transportation is not provided. A total of five child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

3Evidence:
1. The belongings and backpacks of the school age children present for care were on the floor, on chairs, on the table stacked up on top of each other. There was not an individual place for each child's personal belongings.

1. The first aid kit did not include a first aid instructional manual.
2. The director stated that the first aid instructional manual was not in the first aid kit.
Evidence:
10 out of 10 lunch containers were not labeled and/or dated.
