Inspector Notes
The inspector conducted an unannounced monitoring inspection on 03/20/2023 from 10:07am to 2:20pm. There were 96 children present with 25 staff supervising. The inspectors reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, emergencies and nutrition. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. Ten children?s records and ten staff records were reviewed during this inspection. Information gathered during the inspection determined areas of noncompliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

Evidence:
1. The Pre-K classroom had three uncovered electrical outlets located on an extension cord within the reach of children in care.
2. The 2A classroom had five uncovered electrical outlets located on an extension cord out of the reach of children in care.
Evidence:
The toddler classroom diapering area located in both the purple and yellow room, had a broken step trashcan that was used by the teachers to dispose of soiled diapers. The trashcan was not operable which caused the teachers to lift the lid of the trashcan to dispose of the diapers.
Evidence:
Two out of five diaper creams did not have parent authorizations in the child?s record.
Evidence:
There were eight infants whose protective belts were not fastened securely while sitting in high chairs during meal time.
Evidence:
The record of child #1 (enrollment date: 3/13/2023) did not have documentation of viewing proof of the child?s identity and age.
