The center is required to obtain a completed sworn statement prior to the employee's first
day of employment. The sworn statement for S6 was completed one day after the employee?s first day of employment.

The center must request a search of the central registry prior to the employee's first day
of employment.
S2, who had been employed for 8 days, did not have a central registry
search requested.
S3, who had been employed for 37 days, did not have a central registry
search requested.
S4, who had been employed for 1 day, did not have a central registry
search requested.
S5, who had been employed for 1 day, did not have a central registry
search requested.
S6, who had been employed for 4 days, did not have a central registry
search requested.
S7, who had been employed for 3 days, did not have a central registry
search requested.
S8, who had been employed for 1 day, did not have a central registry
search requested.
S9, who had been employed for 13 days, did not have a central registry
search requested.

The center is required to obtain background checks from any state in which the individual
has resided in the preceding five years. S3, employed for more than 4 months, did not
have documentation of requesting a central registry check or sexual offender check from
one state.

Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
The laminated flooring in the Orange, Green, Yellow, and Blue classrooms is peeling and separating from the underlayment causing it to buckle.
Foam water pipe insulation on the hot water radiator pipes in the Red Room is loose and exposing the pipes.
Rust was observed on base of the exterior door frame in the Orange Room (infant room).
The threshold and flooring into the 2-A classroom bathroom is separating, creating a trip hazard.
The bathrooms on the third floor used by children in the 3-C and 3-B classrooms had peeling laminate exposing wood particle board on the sink vanities.

In areas used by preschool age or younger children, electrical outlets shall have protective covers. A power strip observed in the 3-C room had a power strip on the floor with uncovered outlets.

Repeat Violation
The center shall maintain written records of children?s injuries that contain all of the
required information. Injury reports dated 12/ 3,10,11,15, 16,19, 29, 30/2025
did not document the time of parental notification.

Repeat Violation
When food is brought from home the container must be clearly labeled with the child?s name. Multiple thermos bottles were observed in the C-A classroom were not labeled with the children?s names.

Each center shall maintain a record for each staff that contains all the required information. The record for S2 did not document a second employment reference.

Staff who work directly with children shall have within 90 days of the date of employment
current certification in cardiopulmonary resuscitation. The record for S3 did not contain
the certification due by 11/18/2025.
