Inspector Notes

An unannounced monitoring inspection was conducted today between 8:15am and 9:40am. There were 3 children present at the start of the inspection with the Provider and 2 assistant. The children were observed being dropped off, playing freely, receiving bottles, napping and eating breakfast (yogurt, bananas and milk). Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-800-100-A
Based on record review, the provider did not obtain documentation of a physical examination for each child within the first 30 days of attendance.
Evidence: Child #3 (date of enrollment: 3/16/2022) record does not contain a physical exam.
Plan of Correction: I'll text the parents and ask for them to bring me a copy of their physical next week.
Standard 8VAC20-800-60-B
Based on record review, children's records did not contain all required information.
Evidence: Child #4's record did not contain the families medical insurance policy number.
Plan of Correction: I'll have the parents send me a picture of their policy number to update the record today.
Standard 8VAC20-800-730-D
Based on observation and interview, medication was not kept in a locked place using a safe locking method that prevents access by children.
Evidence: The provider confirmed that there was emergency medication at the family day home for a child with allergies. The medication as being stored in the prescription box inside the child's cubby. The medication was not locked to prevent access by children.
Plan of Correction: I'll order a lock bag/box as soon as possible.
Standard 8VAC20-800-90-C
Based on record review, the family day home did not obtain documentation of additional immunizations every 6 months for each child under two years of age.
Evidence: Child #5 (23 months) most recent documented immunizations are from when the child was 6 months old.
Plan of Correction: I know the child have current immunizations, I think the parents just forgot to bring the record. I'll text the parents to ask them to bring a copy by next week.
