During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on March 19, 2026, and completed on March 19, 2026. The on-site inspection began at 2:00 pm and ended at 3:50 pm. The inspector reviewed compliance in the areas listed above. There was one child present and one caregiver. The inspector reviewed one child?s records and five caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the renewal inspection. Subsidy standards that are not covered in voluntary registration standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-250-F
Caregivers who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course. This training shall be related to child safety, child development, health and safety in the family day home environment, and any required department sponsored training. 1. The record of caregiver #1 contained documentation of one and a half hours of annual training when 16 hours were required. 2. The record of caregiver #1 did not contain documentation of completion of the department?s health and safety update training for the year 2025.
Plan of Correction: Per the provider: I will complete the health & safety training and have started additional hours to meet the 16 hours required.
Standard 8VAC20-821-170-B
Household members 14 years of age and older shall request the results of a Virginia child protective services central registry check within seven days of reaching 14 years of age.
The record of household Member #2 did not contain documentation of a request for the search of Virginia child protective central registry being submitted within seven days of turning 14 years of age. The caregiver stated the central registry check has not been requested. Household member #2 is 15 years in age.
Plan of Correction: Per the provider: I will submit the request for the background check by Monday.
Standard 8VAC20-850-110-C
The provider shall meet the criteria on the health and safety checklist at the
time of the initial evaluation or monitoring visit. Household member #1, did not have documentation of a tuberculosis screening.
Plan of Correction: Per the provider: I believe we have completed it. I will email it or have one completed if needed.
