Inspector Notes
An announced initial inspection was conducted on 5/13/22 from 10:00am until 11:20am. The applicant intends to operate a family day home for up to 4 children, ranging in age from 2 years to 12 years. The entire home, to include areas where children will be cared for, bathroom facilities, kitchen and the outside play area were inspected. First aid and emergency supplies, the emergency preparedness plan and required postings were reviewed. Two provider/assistant records were also reviewed. There were no violations identified during the inspection. A six month conditional license is recommended to allow the applicant the opportunity to demonstrate continued compliance with the Standards for Licensed Family Day Homes. All future inspections will be unannounced. The first unannounced inspection will be conducted within 60 days from the date the license is issued.
