During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on January 15, 2025. The on-site inspection began at 10:00am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 17. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site. This inspection included document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
A child's record shall contain the name, address and telephone number for two emergency contacts in the event a parent or guardian cannot be reached in an emergency.
Child #1 who had been in care for over one year does not have the name, address and telephone for a second emergency contact.
Plan of Correction: I will contact the parent and get the information for the second emergency contact. I will make sure to get the information for both emergency contacts before the child starts.
Standard 8VAC20-800-680-A
REPEAT VIOLATION
Caregivers shall wash their hands with liquid soap and warm running water after each diaper change.
Caregiver #2 did not wash their hands after assisting child #1 and #2 with diapering.
Plan of Correction: We will make sure hands are washed after diapering.

Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after diapering.
Child #1 and #2 did not have their hands washed after diapering.
Plan of Correction: We will make sure hands are washed after diapering.

The diapering surface shall be cleaned and sanitized after each use.

The diaper surface was not cleaned and sanitized after each diaper change.
Plan of Correction: I will speak with the other caregiver to ensure the diapering surface is cleaned and sanitized after each use.
