During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 9/15/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 11:00 and ended at 12:00. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 9. The inspector reviewed 3 children?s records and 2 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4One household member is missing the fingerprint check. The household member has turned 18 in the last 2 months.
The provider has attendance sheets for each child and parents sign the children in.One child is signed in for the next 7 days already at the time of the inspection. two other children present are not signed in and were not signed in for Thursday and Friday of last week.
1. Awareness of and responsibility for each child in care, including being
near enough to intervene if needed.
At the beginning of the inspection the provider was in the driveway to secure a gate so she could let her two dogs outside.in the back yard. During this time three children were in the basement out of sight and sound supervision. The children are 18 months old. They were in a space safe for them but left for approximately 5 minutes while the provider took care of her dogs and was unable to be close enough to intervene if needed..
One child in care, Child 2 is missing the parents work information, physicians information and second emergency contact.
