Inspector Notes
An unannounced monitoring inspection was conducted on 4/28/22 from 9:15am - 12:00pm. During the inspection there were 19 children ages five months old through five years old in care with 10 staff. Children were observed participating in various activities in the classrooms, playing outside, and eating lunch. Records were reviewed for five children while at the center and five staff. Medication, emergency procedures, emergency supplies and transportation procedures were also reviewed during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview.

3Evidence:
1. The record for staff #7 did not contain documentation of a completed criminal record check from Pennsylvania. The sworn statement indicated staff #7 had previously lived in Pennsylvania within the last 5 years.
2. Staff #11 (Program Director) confirmed the sex offender registry check and search of the central registry from the State of Pennsylvania.
Evidence:
1. The Licensing Inspector observed a can of sunscreen in a cubby in the Preschool classroom.
2. Staff #11 (Program Director) confirmed that the sunscreen was accessible to the children present in the classroom.

1. There was no document that contained the feeding schedule for each child who is on
formula.
2. Staff #11 (Program Director) confirmed that there was not a feeding schedule for each child
who is on formula available for viewing during the inspection.
