Inspector Notes
The department received a complaint regarding allegations in the areas of supervision on July 9, 2022. The licensing inspector initiated the inspection on July 21, 2022 and concluded the inspection on July 26, 2022. The licensing inspector was onsite July 26, 2022 from approximately 8:15am until 9:45am.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued. Any other violations not related to the complaint but identified during the course of the investigation can also be found on the violation notice.
Violations
6Evidence:
1. At 8:15 the licensing inspector observed five infants with one staff member in the far left classroom.
2. A member of management stated the Infant and Toddler teachers should be combined in the mornings to maintain the required staff-to-child ratio for infants.
Evidence:
The record for Staff #1 (hire date 07/18/2022), Staff #2 (hire date 07/05/2022) and Staff #5 (hire date 04/04/2022) did not contain documentation from a practicing physician or physician assistant was free from any disability that would prevent them from caring for children under their supervision.
Evidence:
Five staff records were reviewed and did not contain documentation of training for daily health screenings. Members of managements stated they typically look of the children that arrive at the center but staff had not been formally training in conducting a daily health screening.
Evidence:
The record for Child #4 (start date 06/06/2022) did not contain documentation of immunizations. Members of management stated before children begin attending the center, a registration record, proof of ID, and a copy of the child?s immunization record is required to be obtained.
Evidence:
1. The record for Staff #1 (hire date 07/18/22) and results from a fingerprint-based criminal history has not been obtained. Staff #1 was observed supervising children during the inspection. A member of management stated Staff #1 completed the search but the results have not been received as of 07/26/2022.
2. The record for Staff #2 (hire date 07/05/2022) contained satisfactory letter based off of a fingerprint-based criminal history dated 07/18/2022.
3. The record for Staff #3 (hire date 04/04/2022) contained satisfactory letter based off of a fingerprint-based criminal history dated 04/15/2022.
4. The record for Staff #4 (hire date 04/18/2022) contained satisfactory letter based off of a fingerprint-based criminal history dated 04/15/2022.
