Inspector Notes
A complaint inspection was conducted Thursday, December 7, 2022 in response to a complaint received by the department on October 20, 2022 regarding staff-to-child ratios and handwashing. The licensing inspector was on site from approximately 10:00am until 11:30pm. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. The preponderance of evidence gathered during the investigation does support the complaint; therefore, the complaint was determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
3receiving care included children from different age brackets, the age of the youngest child in the group was used to determine the staff-to-children ratio that applies to that group.
Evidence:
1. At approximately 10:00am, the licensing inspector observed 17 children with two staff members in one room. The children?s ages ranged from two years to four years of age.
2. Approximately five minutes later, eight children and one staff member went to the Two?s classroom. Nine children remained in the classroom with one staff member. Child #1 remained with the nine children. Child #1 was two years of age. The other eight children were three years or older.
3. A member of management confirmed Child #1 was two years old.
4. The required staff-to -child ratio is one staff member for every eight children present.
Evidence:
1. The licensing inspector observed a staff in the Older Infant room change children?s diapers without washing the staff or children?s hands.
2. The staff member stated hand sanitizer and wipes are used to clean staff and children?s hands after diapering.
Evidence:
1. The records for Child #1 and Child #2 did not have documentation of immunizations.
2. Child #1 and Child #2 were present during the inspection.
3. A member of management confirmed this information while the inspector was on site.
4. The parent handbook states a copy of immunization records is required to be obtained prior to attending the center.
