Inspector Notes

An unannounced monitoring inspection was completed on 04/10/2025 with a local agency in response to a self-report received by the licensing office on 04/07/2025 relating to a serious injury to a child while in care of the center. The inspector reviewed 1 child record and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/28/2025. A POC submitted after this date will not appear on the public website.

The facility shall establish and implement a procedure for appropriate supervision of all children in care to ensure safety of children . S1 while placing a child into a highchair in the Infant II classroom, C1, an infant pulled himself up on the mini refrigerator in the same classroom, and ??grabbed the crockpot cord and yanked it down causing hot water to fall onto him?. C1 received 2nd degree burns to his face, neck, and shoulder requiring hospitalization. The crock pot was located on top of a mini refrigerator and the power cord was accessible to the child.
Plan of Correction: As a facility we will be continuing to enforce, provide, and require teachers to be trained in active and preventative supervision.

The facility is required to ensure that all areas of the premises accessible to children are free of obvious injury hazards. C1 pulled up on a mini refrigerator located in the Infant II classroom that had a crockpot containing hot water used to warm bottles. C1 grabbed the accessible power cord of the crockpot causing it to spill onto him resulting in 2nd degree burns on the left side of C1?s body.
Plan of Correction: Although crockpots are an acceptable form of bottle warming in facilities by the state, and the crockpot was not deemed to be in a hazardous place during previous yearly inspections we have deemed the risk too great and have removed them from use at our facility.
