During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on 10/30/24 and completed on 10/30/24. The on-site inspection began at 11:05am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and 11 staff. The inspector reviewed 6 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Areas of the center, inside and outside, shall be maintained in a safe and operable condition.

On the younger children's playground, there are four slats of the wood fence that are broken and splintered, creating an injury hazard to the children in care.
Plan of Correction: The center responded with the following: A maintenance order has been requested for the fence to be repaired.

Documentation that two references as to character, reputation, and competency are to be checked prior to employment.

Staff 1, who has been actively working at the center for the past seven months, does not have completed reference verifications on file.
Plan of Correction: The center responded with the following: Reference checks for staff 1 were attempted before hire; however, the calls were not returned. Center management has attempted to call the provided references again with no response. Additional references will be obtained for staff 1 and the results will be placed in the staff record.
