Inspector Notes

A complaint was received in the licensing office on February 08, 2023 regarding a three-year-old child's disenrollment and care and protection. On February 10, 2023, an on-site investigation was conducted. Interviews were conducted and observations were made. At the time of this investigation, the preponderance of evidence did not support the allegation of disenrollment of child and care and protection.

Standard 8VAC20-780-340-A
Based on interviews, documentation, and video recordings, staff supervising children did not always ensure their care, protection, and guidance.
Evidence:
1.On January 5, 2023, there were four staff members in the indoor gym with approximately 14 three-year-old children.
2. A child walked up slide. Staff did not come over to the child and provide redirection.
3. Another child was observed sliding down the slide, hit another child in the back who was sitting at the end of the slide. Staff did not come over to the children and provide redirection.
4. The four staff did not intervene or redirect to ensure children safety using the indoor play equipment.
Plan of Correction: Director provided playground safety retraining in January 2023.

Based on review of records and video camera footage, the center did not develop written playground safety procedures.
Evidence:
1. The center did not have a written playground safety procedures to follow to ensure the safety and supervision of children when using playground equipment.
Plan of Correction: Director reach out to corporate and a meeting is scheduled on April 26, 2023 to create a playground written playground safety document.
