During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on December 4, 2024 and completed on December 5, 2024. The inspector reviewed compliance in the areas listed above. There were 105 children present and 22 staff. The inspector reviewed 8 children?s records and 5 staff records on-site on December 4, 2024. This inspection included document review, tour of the facility, observations.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 13, 2024. A POC submitted after this date will not appear on the public website.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #1, who had been enrolled for two months.
Plan of Correction: Center has contacted family to obtain the health care summary. Family will submit
the form within 5 business days.

Center Admin team will ensure that moving forward all children have the
required documents before starting care. Center Director will audit files for compliance before first day of care.
