Inspector Notes

An announced initial inspection was conducted from 11:00 am -12:00 pm. There were no children in care during the time of inspection. The family day home was observed for adequate supplies, activities, and equipment for the ages of children requested for licensure. Emergency supplies and equipment were observed and assessed. Background checks for the provider and one household member were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-480-A
Based on observation and interview it was determined that the family day home did not have sufficient quantity and variety of play materials and equipment readily accessible to children. Evidence: There was not a sufficient quantity and variety of materials for children. Play equipment observed for use in the family day home was two play chairs and two push toys.
Plan of Correction: The provider will purchase a sufficient quantity of equipment and materials for children in care.
Standard 8VAC20-800-480-C
Based on observation and interview it was determined that the required equipment and materials such as arts and crafts materials, texture materials, construction materials, music and sound materials, books, social living equipment and manipulative equipment were not accessible for children. Evidence: There was two play chairs and two push toys available to children.
Plan of Correction: Appropriate supplies, materials, and equipment will be purchased for the family day home.
Standard 8VAC20-800-760-A
Based on observation and interview with the provider it was determined that the required emergency supplies were not accessible in the family day home. Evidence: There was not a first aid kit available in the family day home.
Plan of Correction: The provider has purchased a first aid kit.

Based on observation and interview with the provider it was determined that emergency flashlights were not available in the family day home. Evidence: There was not a flashlight available in the family day home for emergency use.
Plan of Correction: The provider has purchased an emergency flashlight.

Based on observation and interview with the provider it was determined that the required telephone numbers were not posted. Evidence: The telephone numbers for emergency back up care, 911, and poison control were not posted.
