Inspector Notes

An unannounced complaint inspection was conducted on 12/9/2022 to investigate an allegation received by the department related to Programs - Daily activities for infants. Based on evidence gathered through observations, interviews, a review of documentation and a photograph, the complaint was determined to be valid. A violation notice was issued to the facility. Standard 8VAC20-780-370-1
Based on multiple interviews, and a review of a photo, it was determined that the facility did not ensure that when an infant is placed in his crib to sleep, he shall always be placed on his back. Evidence: infant #1 (age three months) was observed (in a photo) sleeping in a crib on their tummy on 11/29/2022 between approximately 10-10:30 am. Staff interviews revealed that they put infant #1 to sleep on their tummy because they sleep better that way. Staff stated that infant #1 is unable to roll over from back to tummy or tummy to back. It was determined that infant #1 was also placed to sleep on their tummy on 11/21/2022 between approximately 1:30 - 2:30 pm.

Standard 8VAC20-780-40-E
Based on interviews with staff, posted procedures in the classroom and review of the center?s procedures for safe sleeping practices and sudden infant death syndrome awareness, the facility failed to follow the center's own policies and procedures that are required by these standards. Evidence: The center?s safe sleep policy, states that "Start by always placing babies on their backs to sleep." Staff admitted that infant #1 (age three months) was placed on their tummy to sleep on more than one occasion. Staff stated that they were aware of the center's safe sleep policy to always lay babies down to sleep on their back in the crib.
