Inspector Notes
An announced initial inspection was conducted today to determine compliance with licensing standards. The inspection was initiated at 10:30am and concluded at 12:00pm. There were 4 children directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, the physical plant, cribs, emergency supplies, and policies were inspected. Required documents were posted and the provider's record was reviewed. Children were observed singing in English and Spanish, reviewing the days of the week and reading books. The home was well supplied with toys and equipment for the children. The backyard offers outdoor play space for the children. Areas of non-compliance are identified on the Violation Notice.
A recommendation will be made for the issuance of a six month Conditional License. Questions about this inspection may be directed to maria.soto@doe.virginia.gov.

The record for Child #1, #2, #3, #4 and #5 were missing the Liability Insurance Declaration form and the Information for Parents form.

The prescription medication for Child #3 was not in the original container with a prescription labeled attached.
